Photo/media release: I give L.A.S.H. Foundation permission to use photos and videos of me taken during volunteer activities on its website, official social media accounts, newsletters and emails, annual and grant reports, and printed materials to promote its mission. My full name will not be used without my separate permission. I may withdraw this permission at any time by emailing support@lashfoundation.org; L.A.S.H. will stop new uses but cannot recall materials already printed or published. This release applies to volunteers only — clients are never photographed under it.

I do NOT give photo permission.

Confidentiality acknowledgement: I will protect confidential information learned through volunteering and will not disclose client information except as authorized or required by law.
